Shrewsbury Nursing & Rehabilitation Center (SNRC) (http://www.shrewsburynursing.com/index.html) is a family-owned, skilled nursing facility located in Shrewsbury, Massachusetts. We specialize in short-term rehabilitation, long-term care, and memory care. SNRC also maintains a strong palliative care practice as well as respite care and hospice care. Founded in 1969, SNRC is located on a 30-acre park-like campus.
We are seeking a Per Diem COTA to provide skilled occupational therapy services consistent with professional standards and policies. Participates in the development of programs necessary to achieve patient care objectives.
EDUCATION Minimum of a bachelor’s degree from an accredited Occupational Therapy program.
EXPERIENCE Prefer a minimum of one year of experience in a therapist capacity.
Preferred experience in home health and/or geriatric/adult outpatient therapy services.
QUALIFICATIONS Licensed as an Occupational Therapist in the state(s) of practice.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES AND STANDARDS
• Provides occupational therapy in accordance with physician's orders and accepted professional standards of practice.
• Performs patient centered evaluations to determine functional level and establish a therapeutic plan of care.
• Develops an overall therapy plan of care to include treatment and medical diagnosis warranting treatment, the planned duration and frequency of therapy to be provided, a minimum of long-term goals and discharge plans, per standards of practice.
• Utilizes standardized assessments upon the development of the plan of care to validate the need for skilled therapy services and demonstrate repetition of this standardized assessment to reflect the ongoing need for skilled therapy and the patient 's progress.
• Completes all documentation required and in accordance with state and federal regulations.
• Documents observations, patient 's response to treatment, treatment programs provided, degree of progress and notable changes in patient's condition.
• Communicates and coordinates patient's care with other members of the patient' s health care team and the patient.
• Provides patient, family and caregiver teaching related to the patient's occupational therapy program, safety, and functional status.
• Assist the patient and/or family to procure needed equipment or supplies for optimal functioning and safety both in and out of the home environment.
• Utilize appropriate infection control practices including adherence of PPE, hand washing, standard precautions and personal protective equipment per state and federal regulations, and company policy.
• Responsible for meeting designated productivity expectations as directed by the Director of Rehabilitation.
