Details:
- The Patient Support Specialist (PSS) works under the supervision of the MMT Program Coordinator to facilitate patients’ access to healthcare services and to provide referral and patient flow support to NHFHS Care Teams. The PSS will work with patients in the community being seen on the MMT. Maintains a library of patient educational materials and forms related to the resources available to assist in patients’ healthcare. Works to process practitioner referral orders in a timely fashion and forwards necessary EHR information as needed and required for each individual order.
Experience and Skills:
Education:
- An Associate’s Degree in Human Services or Health Care field and two years’ experience in medical care setting or
- High School graduate and four years of relevant work experience in a medical care setting.
Knowledge and Skills:
- Must demonstrate thorough knowledge of health / disability insurance and managed care plans
- Must be able to interpret and apply written policies and procedures
- Must have excellent customer service skills
- Must have working knowledge of computer programs and professional construction of emails
- Must demonstrate legible handwriting, excellent spelling, and ability to speak clearly and in a grammatically correct manner
- Must demonstrate the ability to accept responsibility as indicated
- Must be a motivated and enthusiastic learner
- Must present a courteous, tactful and professional manner
