Programs are offered for children, adolescents and adults struggling with mental illness as well as offering services specializing in Anxiety, Eating Disorders, and Geriatric Services.* * *Job Summary* This position ensures patients have been cleared for specialty services office visits. Point person for all hospitals, department and other health care facilities pertaining to questions/concerns on all provider orders/ referrals. Provides support to clinical staff in order to facilitate the administrative components of clinical referrals for various services. Acts as a liaison between hospital, physicians, health plans, vendors and patients, or other referral services. Manages all outgoing referrals to appropriate agencies to assist the client in achieving the goals and objectives identified in their Care Plan. *Job Responsibilities* 1. Gathers pertinent information from insurance carriers, financial counselors, and other ancillary staff to make certain the patient’s financial obligations for services provided. 2. Processes paperwork for new patients efficiently and in accordance with protocols. Verifies insurance coverage and obtains authorizations if needed from insurance carrier. Resolves pre-certification, registration and case-related concerns prior to a patient’s appointment. 3. Tracks and support patients when they obtain services outside the practice e.g. Emergency departments, Hospital and other health care facilities. 4. Monitors and meets PCMH referral processes. 5. Enters referrals and documents communications, actions and other data in an information system. Ensures that all referral requests are properl6y signed and filed. 6. Care coordination – assists with specialty appointments and obtain needed tests recommended by specialist. 7. Communicates with other team members and referral teams constructively in order to reduce conflicts and enhance the resolution of issues. 8. Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns on orders and referrals. 9. Working collaboratively with other team members, provide care management services for high risk patients. 10. Obtains insurance referrals for patients, getting PA for diagnostic imaging and sending referrals to specialist office for cardiac and preventative screenings. 11. Assist and promote preventive care appointments. Tracks Breast cancer screening orders. * Associate's degree or equivalent knowledge in ambulatory environment * Five years' experience working with referral processes * Familiarity with EPC EHR system * Experience in ambulatory healthcare environment with front and back end office process involving patient scheduling, registration, information flow, billing/coding and office systems * Strong verbal and written communication We take great care of careers.
