Environment:
Work will be performed primarily indoors at one of our long-term healthcare facilities, throughout all areas, including in resident rooms, and on carpeted and/or tiled floors. Work will be performed there routinely around other co-workers, healthcare staff, residents, and guests. Due to the nature of facility’s business, worker may be exposed to occasional slippery floors, object on floors, chemicals, sharp objects, hazardous materials, and waste (including human), blood borne pathogens, and communicable diseases, as well as high-stress medical and/or life-threatening situations.
Essential Duties & Responsibilities:
· Visiting New Admissions – Resident Welcoming Process.
· Calling Family Members or Representatives of New Admission – Resident Welcoming Process.
· Completing daily and weekly visits and call to residents and family members – Resident Welcoming Process.
· Scheduling Resident Appointments.
· Scheduling Transportation for Resident Appointments.
· Working with the Staffing Coordinator to schedule escorts for the transportation appointments.
- Works closely with the Director of Admissions, Director of Nursing and Administrator to assess resident needs in the facility.
- Develop and implement programming strategies to serve residents.
- Maintain up to date knowledge on best practices related to service coordination, senior aging in place and family self-sufficiency.
- Work with service coordinators and the resident success team to develop standards around service coordinator program delivery.
- Develop and implement evaluation systems that measure the extent to which Copper Ridge is supporting resident success.
- Evaluate existing partnerships supporting both vulnerable populations and service coordination.
- Assess facility landscapes for potential organizations and partnerships to support vulnerable populations and service coordination.
Job Requirements:
Three or more years of applicable program development/management experience.
Strong ability to communicate and facilitate effectively.
Ability to handle multiple tasks simultaneously and meet designated deadlines.
High degree of accuracy and attention to detail. Excellent organizational and workload prioritizing skills.
Excellent verbal and written English communication skills. Ability to write clearly with proper grammar, spelling, and punctuation.
Ability to organize, facilitate, and manage projects and meetings in an efficient and effective manner.
Flexibility is a key attribute for this position. A willingness to take on additional duties to ensure the success of the team is essential.
Proficiency with basic office equipment and extensive knowledge of MS Office products. Experience working on the Internet and with integrated database software is required.
Ability to work evenings and weekends, as necessary.
Ability to travel between multiple locations, transport residents to appointments and attend off premise events, such as community events and meetings. Must have and maintain a valid Driver's License in the state of residence.
