We are seeking a dedicated and compassionate Patient Care Manager who is a Registered Nurse to join our dynamic healthcare team. This is a permanent, full-time position that involves providing top-tier medical care to our patients. Your role will encompass a wide range of tasks, including assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. You will be working in a diverse and fast-paced environment where no two days are the same. Responsibilities:
1. Perform physical exams and health histories before making critical decisions.
2. Provide health promotion, counseling, and education in a hospice or long term care facility
3. Administer medications, wound care, and numerous other personalized interventions.
4. Interpret patient information and make critical decisions about needed actions.
5. Coordinate care, in collaboration with a wide array of healthcare professionals.
6. Direct and supervise care delivered by other healthcare personnel like LPNs and nurse aides.
7. Conduct research in support of improved practice and patient outcomes.
8. Provide emotional support and compassionate care to patients and their families.
9. Supervise home health aides when such personnel are part of the patient's care team.
10. Administer palliative care as needed to keep the patient comfortable.
Qualifications:
1. Must hold a current, valid RN license in the state of practice.
2. A minimum of 3 years of nursing experience, preferably in a hospice or home health setting.
3. Proven experience as a registered nurse.
4. Excellent knowledge of nursing care methods and procedures in hospice or long term care facilities
5. Excellent knowledge of emergency care.
6. In-depth knowledge of health and safety guidelines and procedures (sanitation, decontamination, etc.) and willingness to follow them at all times.
7. A team player with excellent communication and interpersonal skills.
8. Responsible and compassionate.
9. Outstanding organizational and multi-tasking skills.
10. Patient with excellent problem-solving skills.
