Role Description
This is a full-time on-site role for a Patient Care Coordinator at PARC Home Care and Hospice. The Patient Care Coordinator will be responsible for appointment scheduling, phone etiquette, care coordination, medical terminology, and patient care. Daily tasks include facilitating communication between patients, family members, and healthcare providers to support patient independence and prevent re-hospitalization.
Qualifications

- Knowledge of Medical Terminology
- Strong communication and interpersonal skills
- Attention to detail and organizational skills
- Ability to work effectively in a team setting
- Experience in a healthcare or home care setting is a plus
- Associate's degree in Healthcare Administration or related field
