- Position Summary
- Under direct supervision of the Administrator, the Patient Care Coordinator is accountable for the overallperformance and productivity of the division, while promoting continuity and optimal standards of qualitycare for patients/clients. The Patient Care Coordinator assists in the planning, development, implementationand evaluation of the health care services at the divisional level. The Patient Care Coordinator assures that careis provided in accordance with agency and health system's values, vision, mission, goals, policies andprocedures, and State, Federal and JCAHO guidelines. The Patient Care Coordinator performs admissions andassists with the coordination of patient care and services at the field staff level. He/She will be responsible for the ongoing reviewof current and discharged patient charts to assure compliance with home health requirements, agency philosophy, andestablished policies/standards of professional practice. He/She is responsible for the delivery of patient care; case managementof the assigned patients, supervision of assigned personnel and promoting collaboration among the multi-disciplinary health careteam. In absence of the administrator, the Patient Care Coordinator will be acting administrator.

- Associates in Nursing - Required
- Experience
- 3-5 Years Acute Care or Home Health setting - Preferred
- CQI experience desirable, but not required. Supervisory experience a plus.
- License & Certification
- Professional License with State of Practice - Required
- Basic Life Support (BLS) - Required
- Core Job Functions
- Provides care appropriate to any age-related needs of the patients. Assess and interpret data about the patient’s status in order to identify and provide the appropriate care. Admission and subsequent nursing assessments and supervision activities are completed on patients and participates in the on-call process.
- Plan of care developed and customized based on nursing assessment to include medical and nursing diagnosis, patient specific problems and needs under the supervision of the RN. Facilitates patient and family involvement, discharge planning, teaching needs, measurable outcomes are included in plan of care.
- Maintains knowledge and awareness of home health and legislative issues that impact the future of home care.
- Agency policies and procedures are followed in the care, maintenance and use of all supplies and computer hardware. Maintains reliable transportation and ability to travel as required.
- Completes all required documentation according to Agency policies and procedures. Collaborates with all personnel through patient Team Conference to identify, clarify and respond to the needs of the patient.
- Ensures completion of in-home competency check of all field staff upon hire. Accountable for overall productivity in their area. Accepts the responsibility of acting Administrator when necessary.
