Job Summary
The Patient Care Coordinator will focus on promoting home care services to potential clients and healthcare providers. With your clinical expertise, you will also work clients, their families and health care providers to develop the Individual Service Plan of Care for our clients.
Duties include:
Client Engagement and Relationship Management:
- Building and maintaining relationships with potential clients, which could include patients, families, and caregivers, to educate them about home care services.
- Networking with healthcare providers, such as physicians, hospital staff, and community health organizations, to generate referrals and promote services.
Sales and Marketing:
- Developing and implementing sales strategies tailored to the local market to drive service utilization.
- Conducting presentations and seminars to groups to explain the benefits and features of home care services.
- Preparing marketing materials or collaborating with marketing teams to create informative content about home care services.
Clinical Knowledge and Consultation:
- Utilizing clinical expertise to assist clients and families in understanding the specific medical benefits and appropriateness of home care services.
- Serving as a clinical liaison between patients, families, and healthcare providers to ensure a smooth transition to home care services when needed.
- Providing expert advice and feedback to clients and create care plans.
Customer Service and Support:
- Offering ongoing support and troubleshooting for clients using the service.
- Addressing concerns or questions about patient care plans, treatment options, or home care protocols.
Administrative and Reporting Duties:
- Maintaining accurate records of sales activities, potential client interactions, and outcomes.
- Reporting to senior management on sales progress, potential market opportunities, and challenges within the territory or assigned area.
- Ensuring compliance with healthcare regulations and maintaining the confidentiality of patient information.
Pay
- Base Salary $50,000 + bonus
Location
- Wilmington and New Hanover County
- Willingness to travel in Brunswick and other counties surrounding Wilmington
Qualifications
- Holding a valid registered nurse license.
- Possessing experience in home health care or a related healthcare field is often preferred.
- Sales or marketing experience in a healthcare setting can be advantageous.
