Patient Care Coordinator Job Description
We are looking for a compassionate and dedicated Patient Care Coordinator to join our team at Towner County Medical Center in Cando, ND. As the Patient Care Coordinator, you will work closely with identified high-risk and complex patients, providing guidance and advocacy to improve health outcomes through educating patients, building trust between patients and medical practitioners, and enhancing communication and continuity of care. Focuses on decreasing high ER utilization and hospital re-admission frequencies.
Responsibilities:
- Assess the physical, functional, social, psycho-social, and environmental needs of patients
- Provides clear and concise documentation, reflecting all patient interactions
- Provides education for patients and caregivers as needed, for understanding diagnoses, treatment options, illness progression, wellness and self-care measures
- Participate in the discharge planning procedure for making follow up phone calls to inpatients to discuss health status, medications, appointment, home services, and any problems that may have risen and assist the patient with coordination of health care as needed.
- Actively participates in the facility QI program
- Manages and coordinates with the BCBC-Blue Alliance Program
- Act as a resource for the nursing department-clinic and hospital. Assisting with patient care as needed
- Maintain a notification system for keeping patients up to date on annual physicals, well child exams, Prolia, Depo Provera, paps, colonoscopies, etc
- Update patient charts with any endoscopy and ophthalmic procedures performed at this facility
- Knowledge and understanding of insurance authorization procedures
- Will cover open shifts and vacations in clinic as needed
Requirements:
- Valid RN or LPN license in the state of North Dakota
- Minimum 5 years nursing experience in a clinical setting preferred
- Fluency in Epic's electronic medical record system
- Display a good working knowledge of Microsoft Office programs, including Word, Excel, and Outlook and other related software and computers
- Ability to work effectively in a fast-paced environment
- Excellent customer service skills
