- Position Summary
- Bilingual Patient Care Coordinator organizes and provides support services for patients with individualized needs outside of the clinic’s scope of services under the supervision of the Operations Manager. This includes a variety of responsibilities, including, but not limited to Patient Medication Assistance, Referral Care Coordination, Maternal Care Coordination and general Case Management responsibilities. All activities related to this position are documented in the Electronic Health Systems (EHS), ensuring that the information is available for reporting and for provider review as directed in the policy and procedures manual.
- Minimum Qualifications
- Education
- High School Diploma or equivalent.
- Experience
- One year healthcare experience in patient registration, admissions, or billing preferred. Medical Assistant training preferred.
- Knowledge, Skills, and Abilities
- Must be computer literate.
- Possess strong interpersonal and communication skills. Calm, professional attitude in stressful situations and interactions.
- High level of ethics in maintaining patient confidentiality.
- Able to interpret identification, income, and residency information.
- Ability to maintain courteous and customer oriented demeanor in a crisis situation and acts responsible to resolve or defuse the situation.
- Ability to adapt well to change.
- Ability to work closely with co-workers, other clinic staff and patients.

- Physical and Mental Requirements
- Sits, stands, walks and converses most of the day.
- Ability to operate all general office equipment including telephone, calculator, computer, and printer/fax.
- Exerts up to 10 pounds of force occasionally including pushing, lifting, carrying, and/or pulling objects.
- OSHA Category III: Normal routine involves no exposure to blood, body fluid or tissue and as part of their employment, incumbents are not called upon to perform or assist in emergency care or first aid.
