ProCare LTC Pharmacy is seeking a Full Time Pharmacist for our Chesire, CT location.

A PV2 Pharmacist daily function is to facilitate and verify the fulfillment of prescriptions.

Salary range: Contingent on experience

Schedule: 1:00pm-9:30pm; 3:30pm-12am rotating shifts with weekend and Holiday Rotation

Job Summary:

1. Check and review Floor Technician work (PV2).
2. Check, verify and replace contents of E-Kits.
3. Pull control medication to match that of label.
4. Maintain count accuracy and organization of the control room.
5. Compound sterile and non-sterile products.
6. Make necessary NDC changes.
7. Process new and completed backorders.
8. Fill and review OOP/LOA requests as they come in.
9. Fill Clozapine orders.
10. Check medications that have been packaged (pre-pack).
11. Supervising other Pharmacists (when PIC) and overseeing Floor Technicians.
12. Assist pharmacy technicians as needed.?
13. On-Call when scheduled.
14. Ensure PV2 is meeting its commitments for pharmacy run times and exceeding expectations of our customers/ colleagues performing PV1.
15. Abide by all federal and state laws pertaining to the practice of pharmacy.?
16. Follow the pharmacy policies and procedures to ensure safe and effective medication use, including participation in the pharmacy’s continuous quality improvement program.
17. Report medication errors to management as they occur.?
18. Knowledge of medication, interactions, and dosage.
19. Complete QA sheets.
20. Perform other duties as assigned.
21. Flex into PV1 role as available.

Qualifications:?

Licensed by the State Board of Pharmacy.

Prior experience in long term care and with Frameworks and Docutrack information systems is strongly preferred.
