Compensation:  $20.35/hour

Position Overview: 

Title:  Outreach Case Manager

Department:  AXS Social Rehabilitation (Young Adults)

Classification:  Non-Exempt; Union

Schedule:  Mon-Fri, 8am-4pm (1st shift)

The AXS Outreach Case Manager will work with Team AXS in the areas of drop-in, addictions community support, mental health, housing and daily living skill. This position will directly case manage young adults in the AXS Center as well as outreach to and case manage young adults residing in SCSI's Residential Programs.  A key area of focus is to connect with young people who are facing significant barriers in their lives. This role requires significant relationship building/engaging skills, the ability to meet young people where they are, and to support them to identify goals in an effort to make positive changes in their lives. This position requires a dynamic individual with significant skills in the areas of advocacy, crisis intervention and conflict resolution. In addition to assisting with the creation of a “community” environment, the successful candidate will be responsible for engaging young people in developing creative programs for AXS within an integrated service delivery model. This position is also responsible for providing support and linkages to appropriate programs and services as required.  

Essential Functions:

Provide Intensive Support/Outreach Services:

• Hold a caseload between 10-15 AXS Members who are in need of outreach case management in the community

• Build rapport with participants
• Ensure individual support plan is implemented in such areas as: daily living skills, education/employment, social supports, health, mental health, addictions, housing, physical health, harm reduction, leisure and recreation, etc.
• Connect young people to community resources
• Work collaboratively with community agencies
• Advocate for all young people involved in the AXS program

Individual Support Planning:

• Complete all daily documentation
• Collaborate and consult regarding strategies in supporting young people to achieve their goals
• Collaborate on AXS Goal Plans with all members receiving AXS supports

Crisis Prevention/Intervention including:

• Identify potential crisis/implementing support and safety plans
• Respond to crises as they arise
• Report all crises to the Program Coordinator, or designee, in a timely manner

Outcomes Evaluation:

• Complete daily documentation that will be used for Outcome Evaluations as needed

Direct Service Delivery:

• Serve as first point of contact for many young adults living on the street or in need of support
• Work with youth to help them identify the steps to achieve their goals and connect them to the AXS Case Manager for connection to appropriate services
• Ensure appropriate monitoring of young people accessing support through the drop-in program

• Focus on meeting young people “where they are” in a relaxed, welcoming environment
• Respond to youth needs, flexible, creative and intuitive with highly developed interpersonal skills
• Is responsible for identifying resource materials and opportunities for the drop-in program
• Ensure that young people are aware and understand the expectation/standards of the program
• Participate jointly in program delivery
• Ensure the maintenance of supplies used for Outreach Programs including the basic needs portion of the program, which focuses on meeting the needs of youth who are living on the streets or have recently left their home, providing food, toiletries, and clothing to young people in need
• Help youth connect to financial support by helping them obtain proper identification, fill out the proper forms and provide transportation and support to attend appointments and meetings to get their lives on track
• Assist youth in obtaining information, or making referrals, concerning choices or goals they identify (i.e. Addictions Services, Mental Health Support, Employment and Education Programs, Community Support, Housing, etc.)
• Attend team meeting and other duties as assigned
• Perform all other duties as assigned

Qualifications:

• High School diploma (or equivalent); Associate's degree, or higher, preferred
• Minimum of two (2) years of behavioral health/human services-related experience
• (custodial care will not be counted as experience)
• Extensive knowledge of youth related issues and resources including housing, education/employment, mental health/addictions, trauma, and criminal justice system
• Commitment to the philosophy of youth empowerment is a requirement
• Current valid driver’s license and vehicle available for use during the workday is required
• CPR/First Aid certified or eligible.
• Bilingual Spanish/English preferred

Physical Requirements:

• Regularly required to sit, use hands to finger, handle or feel objects, tools, or controls, and climb stairs
• Regularly required to walk, talk and/or hear
• Speaking and hearing ability should be sufficient to communicate effectively by phone or in person at normal volumes
• Vision adequate to read correspondence, computer screen, forms, etc.
• Specific vision abilities required by this position include the ability to adjust focus
• Occasionally required to stand and climb or balance
• Occasionally lift and/or move up to 25 lbs
