Job Summary:
The Providence Center Outreach Case Manager will work within both the Access and Emergency Services Departments to provide behavioral health and case management to clients at The Providence Center and in the community.
Coordinate services for consumers to minimize stressors, facilitate the reduction of psychiatric symptoms, and address the social factors of health. Provide education, resources and support to consumers as appropriate
Duties and Responsibilities:
Coordinate a variety of services:
Treat consumers with respect.
Provide supportive counseling, problem solving, and limit setting.
Establish attainable goals with the consumer.
Involve consumers, agencies and others in planning.
Assist clients in deciding which areas they want to address.
Assist consumers with basic medical procedures and hygiene needs.
Encourage consumers to attain the highest possible levels of independence.
Accept the values and lifestyles of consumers.
Use a flexible approach with a diverse population.
Participate as a member of a multi-disciplinary team in the development and implementation of therapeutic services, to include working with children/families who have severe persistent mental illness and/or substance abuse issues, as it relates to case management.
Accompany consumers to appointments with agencies, clinics, doctors, etc.
Initiate vocational/job development activities with consumer.
Monitor client status during spend down process, complete necessary DHS applications.
Assist consumers with benefits acquisition (social security supplemental insurance (SSI), food stamps, advocates office, medical doctor).
Assist to address barriers to engagement in treatment and improve social determinants of health.
In cooperation with managers/access and emergency services staff, assist with assessments, crisis interventions, warm hand-offs, and risk management as needed.
Assess consumers' unmet needs (psychological, physical, housing, and psychiatric).
Identify and respond to crisis situations with crisis clinicians as indicated.
Perform in a calm and professional manner during crisis or emergency situations.
Assist consumers in the access department with screenings and measures that may precipitate their assessment with a clinician.
Assist consumers in locating their treatment providers, including, but not limited to warm hand-offs following intake assessment.
Negotiate and advocate on behalf of the consumer, as needed, with landlords, courts, families, the treatment team, and consumers themselves.
Transport consumers in own vehicle or Center van to planned activities and appointments.
Complete recordkeeping in accordance with Health Information Services (HIS) and other Center requirements within required time frames and abide by Center confidentiality policy.
Maintain cooperative relationships with Center staff, consumers, community agencies and the public.
Attend trainings, case presentations and conferences.
Requirements:
Associate degree required; Bachelor’s degree from accredited four-year College or University in Human Services or related field preferred.
Experience in Human Services Field preferred.
Bilingual Spanish preferred.
Must have valid driver's license and insured auto.
