POSITION SUMMARY
The purpose of this position is to serve in the day-to-day operations of the outpatient pharmacy services. This role encompasses providing direct clinical pharmacy services with supervisory/managerial oversight of all function of the outpatient pharmacy. The incumbent is responsible for the outpatient pharmacy program which includes: management and administration of personnel policies and procedures, providing ongoing analysis of the outpatient pharmacy department to develop programs with the highest quality of care, and to direct and manage programs and activities of the outpatient pharmacy in a manner that ensures the mission, vision, and values of TCRHCC. The incumbent provides technical and administrative supervision to pharmacy team members comprised of clinical and technical employees, Pharmacist, Pharmacy Technicians, and Pharmacy Billers and aides.
Qualifications:NECESSARY QUALIFICATIONS
Education:
Doctor of Pharmacy (PharmD) from a school recognized by the American Council of Pharmaceutical Education
License/Certification
- Must have a current valid licensed to practice Pharmacy in a State, Territory of the United States, or the District of Columbia and maintained throughout employment in good standing
- Must have and maintain a current valid certification of the following throughout employment.
- Basic Life Support (BLS) Certification with American Heart Association (AHA)
Experience:
- Three (3) years of experience as a pharmacist in a retail/ ambulatory care pharmacy setting
- One (1) year supervisory experience

Physical:
Must be able to walk, bend, twist, maintain balance, reach for a prolonged period of time, sit, stand, kneel, and crouch frequently, and occasionally drive and climb. Must have ability to lift up to 10lbs for a prolonged period of time, push and pull up to 34lbs for a prolonged period of time and occasionally lift, push and pull over 100lbs when necessary. Must have ability of far, near, color vision, depth perception, seeing fine details, hearing normal speech, hearing overhead pages and telephone use. Must have ability of both hand manipulation in simple and form grasping, fine manipulation and use of keyboard.
Mental:
The employee identifies deficiencies in program operations and pharmacy services and makes recommendations for change in procedures and/or guidelines to ensure the delivery of quality patient care services. Must be able to cope with high level of stress, make decisions under high pressure, cope with anger/fear/hostility of others in a calm way, manage altercations, concentrate, handle a high degree of flexibility, handle multiple priorities in stressful situation, work alone, demonstrate high degree of patience, adapt to shift work, and work in areas that are close and crowded. Environmental:

Responsibilities:ESSENTIAL FUNCTIONS:
- Performs job responsibilities in accordance with organization and departmental policies, procedures, performance improvement objectives, environmental and infection control standards, as well as state and federal laws and regulations
- Ensures computer order entry is accurate, thorough and completed in a timely manner. Reviews and updates medication profiles and is cognizant of patient charges
- Reviews medication orders for therapeutic appropriateness, duplications, interactions, overall effectiveness and safety. Seeks clarification if necessary and documents interventions.
- Participates in cost effective use and selection of pharmaceuticals to meet budgetary goals.
- Communicates drug information to physicians, nurses, etc. in courteous and professional manner.
- Assist in patient and family education and counseling regarding drug therapy and side effects
- Documents clinical activities, demonstrating cost savings, ensures staff utilize clinical monitoring tools and maintain documentation
- Conveys key information required for efficient operations and up to date standards.
- Manages pharmacists, technicians and other supportive personnel; ; applies organization and departmental polices in daily practice and holds staff accountable
- Responsible for coordinating all scheduling needs of the service unit
- Finds opportunities and is proactive in improving efficiency and increasing patient safety
- Delegates appropriate and reasonable tasks to pharmacists and technicians and validates accuracy of floor stock replacements and drug doses dispensed from pharmacy.
- Delegates tasks to automation technician and narcotic technician to assist in management of the pharmacy
- Audits and signs out all controlled substances in accordance with state and federal guidelines and organizational policies.
- Investigates and corrects discrepancies and unusual usage patterns and trends, and maintains accurate documentation.
- Participates in clinical programs.
- Participates P&T initiatives and prepares pertinent P&T reports
- Prepares monthly productivity and cost metrics for retail pharmacy functions.
- Sets benchmark productivity statistics for retail and clinical pharmacy functions
- Prepares a comprehensive quarterly ambulatory pharmacy clinic report
- Participates in strategic planning to increase quality, expand service lines, decrease cost and increase employee engagement
- Provides mentorship to pharmacists and technicians
- Monitors target drugs for protocol discontinue, substitution, etc.
- Collects applicable data and measures and communicates opportunities for improvement
- Performs therapeutic monitoring of specified drug therapies and recommends dosage adjustments as needed based on renal function evaluation and pharmacokinetic calculations
- Develops and implements outpatient pharmacy quality assurance activities using predetermined established AAAHC, the Joint Commission, and CMS standards
- Works with performance improvement committee groups to establish, implement and coordinate PI programs.
- As part of the dispensing process, instructs all outpatients on the proper use actions, precautions and storage of their medications. Such activities are documented in the patient’s health record.
- Oversee and responsible for all controlled substances compliance and security, coordinates purchasing and reconciliation of narcotics
- Responsible for electronic health records data entry pertinent to patient service role.
- Acts as a liaison for outpatient pharmacy and other departments in the hospital
- Formulates local outpatient pharmacy policies and procedures, interpreting pharmacy administrative policies, directives and procedures to employees, patients and pharmacy administration.
- Formulates, implements, coordinates, and supervises all outpatient pharmacy related activities within SSU.
- Fosters the interest of the staff in clinical pharmacy activities and monitors the established outpatient pharmacy programs, patient consultation, drug therapy monitoring, pharmacy primary care (acute and chronic), and pharmacy quality assurance.
- Develops and implements outpatient pharmacy quality assurance activities using predetermined established AAAHC, the Joint Commission, and CMS standards.
- Works with performance improvement committee groups to establish, implement and coordinate PI programs.
- Serves on various hospital-wide committees as a representative of the Pharmacy department. Works with leadership team to perform Drug Utilization Review Reports and presents the review to the quality assurance committee through the Pharmacy and Therapeutic Committee as needed.
- Identifies quality and performance improvement opportunities and works with leadership to make positive change.
- Competent in coordination of outpatient ordering in absence of pharmacy purchaser
- Competent in upkeep, operations and management, and utilization of outpatient pharmacy automation including but not limited to Pyxis C2 safe, Accuvax, Propharm, Parata, Pharmacy keeper; able to teach other team members utilization of these automation tools
- Communicates effectively and appropriately with healthcare providers and caregivers (physicians, nurses, tec) and assures continuity of pharmaceutical care between shifts and among staff.
- Ensure proper PPE is worn at all times while on duty including but not limited to, face mask, gloves, gown, isolation gown, NIOSH-approved N95 filtering facepiece respirator or higher, if available), and eye or face shield.
- Complete all donning and doffing tasks in a safe acceptable method and discard of used PPE accordingly. (See CDC website for most current updates).
- Complete task training for all routing cleaning and decontamination processes for all surfaces contaminated by a communicable disease to ensure a high level or patient, visitor, employee and external customer satisfaction.
- Will serve as pharmacy admin on call on a recurring basis.
