Job Summary and Qualifications
Under general supervision of the AVP of Musculoskeletal Services in collaboration Surgical Services, the Ortho/Spine Care Coordinator provides patient care in accordance with established policies and procedures of Medical Center of Lewisville. In collaboration with the medical staff, patients, patient’s family and other members of the health care team, the coordinator is responsible for utilizing a holistic and interdisciplinary approach to coordinate the care of all orthopedic patients. In addition, the coordinator may also be responsible for assessing the therapy needs of patients (and in collaboration with Nursing Services, assessing the nursing needs of patients), planning interventions and evaluating outcomes of care. The coordinator collaborates with Nursing/Rehab Management in addition to providing oversight in their absence and provides direct care within their scope and practice and delegate/collaborate care with other disciplines and non-licensed staff as it relates to the outcome and care of the orthopedic patient. The Ortho/Spine Care Coordinator is responsible for providing care consistent with Medical City Lewisville’s policies, procedures and standards of practice.
Job Duties and Responsibilities:
- Oversees continuum of care for the musculoskeletal patients (orthopedic and neurosurgery). Including Emergency Department, Pre-admit Testing, Day Surgery, OR, Recovery, Surgical Services, PCU, and ICU.
- Serves as a leader to set direction and change in an effort provide care of distinction to the musculoskeletal population (Orthopedic/Neurosurgery)
- Performance management of Musculoskeletal Service Line processes and communicates goals and status to staff and counterparts
- Evaluation and analysis of quantitative and qualitative data related to the Musculoskeletal Service Line clinical outcomes
- Collaborates with physician regarding daily rounds by providing an update such as critical item reports for each patient, etc. (ie. key information as received during nursing report and team meetings) to enhance effectiveness of clinical rounds.
- Makes clinical rounds on patients and appropriate team members on a daily basis and assesses the needs of orthopedic patients from admission through discharge. (Provides education of team member roles, notifies healthcare team of patient needs and variances, ensures problems are addressed and pathways are clear. Collaborates with other members of the healthcare team to ensure care continuum is streamlined and effective for the orthopedic patient. Coordinates discharges with healthcare team and ensures that necessary discharge teaching is complete, goals are met, and follow up discharge appointments are made with physicians, rehab, etc.)
- Integrates Patient/Family concerns into plan of care.
- Acts as a clinical consultant regarding care of orthopedic patients within and outside of the institution- determines appropriate resources and evaluates care, best practices, and aligns with current process to help advance the service line.
- Oversees, assess’, plans, develops, and implements formal and informal patient/family educational opportunities and materials to ensure educational needs are met - includes coordination/scheduling of pre-surgery education classes and community education programs for the orthopedic patient. (Provides annual review of all materials.) Done in collaboration with orthopedic care team.
- Obtains weekly or advance surgery schedules from physician offices and schedules patients for pre-op education classes to ensure patients receive all pre-op materials and information.
- Calls patient prior to surgery if unable to attend a hospital class (provides information and encourages meeting with patient day of surgery at check in to assist in education and inform of expectations during stay).
- Makes pre-operative phone call to patient prior to surgery and provides education about procedure, post op care and discharge planning.
- Coordinates and Assists the Preoperative Education Class (ie Joint Replacement) and varies schedule to meet the patient population with the Interdisciplinary team.
- Consultation visit with patient and/or family in Surgery department prior to or during procedure to answer any questions, provide family materials and contact information.
- Develops & coordinates education and re-education of staff who provide care for orthopedic patient population. (Assists clinical unit educator by reviewing orientation materials, checklists, and competencies, etc. that directly affect the orthopedic patient)
- Coordinates the care continuum of the Musculoskeletal Patient with staff and physician within the service line (participates and collaborates with interdisciplinary service line team to formulate, implement, and evaluate protocols and procedures affecting the plan of care for the orthopedic patient to ensure that successful processes are in place.
- Promotes “Joint Connection” activities such as group therapy as appropriate, group meals, etc. to enhance participation and achievement through pathways.
- Acts as a liaison to the patient/family, physician, and hospital team members that are involved with the orthopedic patient. (Coordinates Health Fairs, Community Seminars, etc. and serves as a liaison for the hospital and community providing awareness to the orthopedic population served regarding services offered at Medical Center of Lewisville.)
- Participates in weekly interdisciplinary discharge planning meetings related to the Musculoskeletal Patient to ensure team conferences include review of patient care, treatment interventions, and services. Also ensures team members are accountable to address goals/initiatives that were addressed during individual clinical rounds.
- Evaluates and provides continuous quality improvement in orthopedic care to ensure quality of program throughout LOS. Works with healthcare team to follow up on variances.
- Implements practices that directly reduce/improve LOS
- Collaborates with Leadership to develop and implement an annual strategic marketing plan, and assists in sharing measured successes to orthopedic surgeons, referring physicians, and the community.
- Promotes utilization of orthopedic service line by serving as a resource to referring physicians, collaborating with others to make site visits to appropriate healthcare providers, providing educational programs based on requests and assessed needs, identifying resources for providing care, and maintaining communication and collaboration with varied healthcare providers. (identifies opportunities to enhance current market share and referral patterns)
- Assists in the Quality Improvement Process by maintaining Musculoskeletal Service Line Dashboard (ie. monthly/quarterly admissions, outcomes, LOS, denial of days, complications and co-morbidities, other HMI data, departmental PI, and hospital PI, etc.) and develops action plans to address any reported variances. (provides feedback and accountability to hard-wire the orthopedic care philosophy and is a resource to Orient Staff, Re-train Staff, and sustain the initiatives and excellence in the musculoskeletal service line)
- Manages SAT (Satisfaction Action Team) and tracks satisfaction reports (develops action plans to drive results that directly affect the Musculoskeletal Patient throughout their stay)
- Manage post discharge calls weekly (includes calls to post discharge institutions, home, etc. to ensure quality of referral sources for orthopedic patients)
- Maintains database of patients and call schedule for follow up at appropriate intervals up to 6 months. (includes obtaining and maintaining functional status data, HOOS/KOOS tool, Oswestry tool, perception of improvement, and quality of life data, etc).
- Coordinates graduate dinners. (Joint/Spine/etc.)
- Infrastructure development of Joint Connection Support Group and selection of key advisors to promote contact with patients as necessary.
- Develops documentation tool for documenting nursing care/interventions in compliance with policies procedures and practices established by the Nursing Department and the hospital.
- Performs patient care consistent with the needs of adolescent, adult and geriatric patients requiring treatment and care within scope of professional license.
- Attend staff meetings and in-services.
- Maintains clinical competencies in the designated patient care area
- Manages Ortho Service Line Marketing programs
- Maintains involvement in hospital-wide committees and various hospital leadership activities to ensure collaboration with other departments and service line awareness.
- Manages DSC for the service line including data management and facility awareness
- Supports Rehab management as oversight of operations in their absence.
What qualifications you will need:
- Current Texas RN licensure - RN or RN Compact State License OR Current Texas PT/OT licensure
- BCLS certification
- Clinical experience in the care and management of the orthopedic patient preferred
- Advanced Certification in Acute Orthopedics or related certification recommended (but not required)
- Must have minimum of 5 years recent clinical nursing or therapy experience in area of practice related to orthopedics
