Clinical Specialist - Solid Organ Transplant
Day shift with transplant pharmacy weekend coverage.
This position will rotate through the following areas: Adult in-patient transplant services: Kidney, Liver, and Lung; Transplant Clinic; and Pediatric abdominal transplant (Kidney and Liver) Expectations of this position are as follows:
- Deliver all aspects of programmatic clinical services for SOT patients with emphasis on best practices, safety and quality initiatives, therapeutic drug monitoring, and disease state management
- Provide comprehensive clinical pharmacy services for SOT patients to optimize safe, effective and cost
- Effective drug therapy including: Multi-disciplinary patient care rounds (daily), Pharmacotherapy consults o Patient, provider and nursing education
- Continue to cultivate integrated model within specialty pharmacy
- Maintain cross-coverage competency for all SOT areas
- In coordination with the SOT team, provide coverage for scheduled days off, paid time off, administrative days, and unplanned absences for all SOT shifts as listed above
- Precept pharmacist Interns and/or pharmacy residents on SOT learning experiences
- Provide support for and participate in Pharmacy & Therapeutic Sub-Committee(s)
- Actively participate in the Transplant Pharmacy Group, and assume ownership towards achieving goals and objectives established by the group and departmental leadership
- Participate in transplant, and pharmacy specific meetings when needed o Clinical Management team, listing, staff meetings, quality meetings, etc.
Qualifications:
Required:
- Pharmacy licensure in the state of Texas and a B.S. Pharmacy or PharmD degree , or eligibility for licensure at time of application o Pharmacy Preceptor license, or eligibility for licensure, in the state of Texas o Completion of an ASHP-accredited PGY1 Pharmacy Residency and PGY2 Solid Organ Transplant residency, or equivalent experience
- Equivalent experience = PGY1 + 3 years SOT experience, or 5 years of SOT experience.
- Equivalent training to residency. i.e. fellowship in SOT o Board certification, or eligibility to test for board certification Preferred: Board Certification in Solid Organ Transplant Pharmacotherapy (BCTXP), or eligibility to test
