On-call Service Coordinator - Vibrant Health Homecare

CURRENTLY HIRING FOR:

We're looking for an on-call service coordinator who is responsible for being the agency's contact person after hours and on the weekend. 

RESPONSIBILITIES

• Answer phone inquiries from employees and clients and provide solutions per company policy.

• Perform home visits with clients who receive care on the weekend.

• Perform scheduled admin tasks. 

• Coordinating services and resources to meet client needs

• Communicating with clients, service providers, and other stakeholders

• Monitoring and evaluating the effectiveness of services

• Advocating for clients and ensuring their rights are upheld

• Providing support and guidance to clients throughout the service process

• Maintaining accurate and up-to-date records of client information and service delivery

Schedule:

• Monday - Thursday 5pm - 9am

• Friday 5pm - Monday 9am

• Saturday and Sunday 24hrs

QUALIFICATIONS

• CNA or HCA license is required.

• Previous experience with care coordination is also required.

• Ability to treat clients with dignity and respect

• Ability to interpret the client’s provided plan of care

• Valid driver’s license and transportation

• Ability to be flexible and adapt to new situations

• Prior experience with senior care and individuals with disabilities is desired and not required
