ESSENTIAL DUTIES:
1. Analyzes department related information regarding operations, budgets, staffing reports, and/or other activities, and develops action plans based on identified trends and variances.
2. Creates standard work for all System Wide Care Managers.
3. Establishes SWCM KPIs, effectively communicates and monitors KPIs in alignment with Genesis System initiatives and scorecard metrics and GIHN objectives.
4. Works collaboratively with leaders in the organization in a dyad operating model to assure success with the SWCM team.
5. Demonstrates understanding of operational issues in areas of responsibility providing support as needed for staff.
6. Participates in interdisciplinary development of evidence-based patient standards of care and the collection and analysis of patient outcomes. Includes facilitating participation of staff in same.
7. Develops, implements, and evaluates systems and processes that complement the overall system for performance improvement: uses data to drive decisions, plan and implement performance improvement strategies for case management.
8. Actively represents Care Management on hospital committees, GIHN committees and teams and facilitates opportunities for employees to do the same.

QUALIFICATIONS:
1. BSN with current RN license in the state of Ohio, or Master of Social Work/Counseling with current license in the state of Ohio.
2. Three (3) years of work experience in a hospital or inpatient care setting.
3. Three (3) years of management or demonstrated leadership experience required.
4. Knowledge of clinically integrated networks, prospective payment systems, managed care, infection control surveillance, patient care, disease processes, discharge planning and continuum of services offered within Genesis and externally.
