Your Role:
As an Occupational Therapist, you’ll deliver comprehensive, high-quality care to clients in their homes, following best practices and maintaining compliance with agency policies. This role will support patients in both Onondaga and Oswego County.
What You’ll Do:
- Perform client assessments and develop tailored plans of care.
- Provide safe, effective therapy interventions in a home setting.
- Collaborate with an interdisciplinary team to ensure optimal client outcomes.
- Maintain accurate and timely documentation, including OASIS requirements.
- Supervise paraprofessional staff involved in client care.
- Participate in ongoing professional development and quality improvement initiatives.
Performance Standards and Responsibilities:
- Ensure quality and safe delivery of home health services.
- Provide client assessments using best clinical practice.
- Develop appropriate plan of care.
- Provide safe occupational therapy interventions to clients/families in accordance with agency policies and procedures.
- Maintain client care records using approved documentation and format.
- Documentation reflects occupational therapy care and interactions.
- Documentation is clear and concise.
- Documentation is completed in a timely manner.
- OASIS documentation is completed within regulatory time lines.
- Follow professional code of ethics and standards of practice.
- Manage a diverse client caseload.
- Communicate client care issues to interdisciplinary team
- Assume responsibility for supervision of paraprofessionals relevant to patient care plan.
- Monitor job performance of home health aides and delivery of home health aide services.
- Comply with VNA policies and procedures.
- Maintain VNA productivity standards.
- Maintain adherence to agency policies and procedures including Corporate Compliance, Ethical Standards, and Confidentiality (HIPAA).
- Demonstrate knowledge of federal, state and local government rules and regulations.
- Demonstrate knowledge and use of infection control practices.
- Adhere to uniform policy.
- Adhere to quality improvement standards.
- Keep personnel requirements up to date, i.e. health requirements, professional license requirement, professional liability insurance and other VNA requirements.
- Demonstrate skills competency through periodic evaluations.
- Participate in personal growth and development.
- Participate in education in-services and meetings.
- Maintain and update clinical knowledge on an ongoing basis.
- Present a professional image to clients and the community.
Requirements:
- Currently registered by New York State and the American Occupational Therapy Association.
- Current New York State Occupational Therapy license.
- Current New York State driver's license and automobile insurance.
- Current professional liability insurance
- One year experience as an occupational therapist preferred.
Physical Requirements:
- Speaking/visual/hearing ability sufficient to comprehend and express written/verbal communication.
- Ability to perform tasks involving physical activity including:

- Pushing/pulling - Typical weight 50 pounds, Maximum weight 150 pounds
- Lifting and carrying - Typical weight 25 pounds, Maximum weight 40 pounds
- Patient transfers, extensive bending, standing, kneeling, walking in various weather conditions e.g. snow, rain, sleet, and going up and down stairs.
- Ability to drive on a daily basis in all weather conditions to Onondaga and/or Oswego County.
Compensation & Benefits:
Competitive Salary [This position is an S03 exempt position with a min-max rate of: $72,415.71- $83,922.76 /annually]with:
