Overview: Our Award Winning Client, a nationally operating organization renowned for its excellence, is seeking an Occupational Therapist to join their team. The ideal candidate will have experience in Occupational Medicine, Social Work, and possess a valid Occupational Therapist, Registered (OTR) license in Illinois (IL). Additionally, candidates must hold a valid Regular Driver License (DL) and possess any of the following valid licenses/certifications: American Heart Association Basic Life Support (AHA BLS), American Heart Association Advanced Cardiac Life Support (AHA ACLS), or American Heart Association Pediatric Advanced Life Support (AHA PALS).

Key Responsibilities:

• Perform assessments of patient functioning levels and develop short and long-term goals towards improving/restoring independence and physical/cognitive functioning.
• Provide guidance and instruction to patients/family members on therapeutic procedures/activities and adaptive approaches.
• Conduct functional and cognitive re-evaluations to assess the need for revisions in treatment plans.
• Maintain progress notes and submit all required paperwork in a timely manner according to Agency policy.
• Arrange for outpatient occupational therapy services as necessary and provide consultation to the home health team.
• Prepare periodic reports for physicians and third-party payers regarding care provided.

Schedule and Shift Details: The work schedule and shift expectations are based on assigned caseload. Employee will be required to drive daily for meetings and/or visits to residential homes.

Travel: Employee will be required to drive daily for meetings and/or visits to residential homes. Occasional travel may be required.
