1. General job duties. Types of procedures required.
Responsible for providing and coordinating comprehensive care to patients of all ages through the proper
utilization of evaluation, planning and implementation of a treatment plan in accordance with established
standards, policies, procedures, protocols and guidelines. Embodies the Memorial Health System Performance
Excellence Standards of Safety, Courtesy, Quality, and Efficiency that support our mission, vision and values.
1. Provides rehabilitative care to patients of all ages
2. Evaluates treatment needs of assigned physician referred patients of all ages.
3. Determines and implements plan to accomplish treatment goals within appropriate time frames.
4. Provides appropriate feedback to physician, department, and other team members and maintains required documentation.
5. Embodies the Memorial Health System Performance Excellence Standards of Safety, Courtesy, Quality, and Efficiency that support our mission, vision and values:

6. Maintains skills in treatment procedure through utilization of inservice and continuing education opportunities and expertise of other clinical staff personnel.

10. Promotes health and safety in the work environment.
11. Actively participates in competency assessment process.

2. Equipment utilized in the unit/dept. (Is there computerized charting, medication systems, etc.)
CPSI, WebPT
3. Required skills and minimum years of experience.
Experience:
Experience with neurologic and orthopedic injuries desirable.
Other Knowledge/Skills/Abilities:
Basic Knowledge in occupational therapy practices in accordance with O.T. Practice Act.
Universal Precautions Category I.
Must possess valid Illinois driver s license and must be deemed as an acceptable driver in accordance with the
MHS Fleet Safety Policy (five year MVR will be required). Must show proof of automobile insurance
4. Specific licensures and certifications/registrations required. (ARRT, CRT, BLS, ACLS, etc)
Education:
Graduate of an approved school of Occupational Therapy
Licensure/Certification/Registry:
Illinois licensed/licensure eligible.
CPR Certification is required.
5. Shift or schedule requirements. (Will the employee ever work alone? Is call required? If so, how often? Are weekends required?)
M-F 8:00 AM to 4:30 PM. No call and no weekends.
6. Floating to other units/departments. (If so, to which units/departments?)
Potential for floating to outpatient (~10% outpatient, ~90% inpatient)
7. Bed size/number of rooms for this unit/dept.
