Job DescriptionThe Occupational Therapist reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
Responsibilities:
- Completes initial exam and establishes appropriate, functional, and objective goals based upon clinical examination and interpretation of objective tests to determine accurate clinical diagnoses and treatment plan in conjunction with MD.
- Develops and implements occupational therapy programs aimed at maximizing function and assisting the patient to adapt to any residual disability by utilizing practice in functional, prevocational, vocational activities. This would include (but not limited to): homemaking skills; Activities of Daily Living, sensor motor, educational, recreational and social activities.
- Promotes maximum independence by selecting and constructing therapy activities according to the patients abilities.
- Communicates plan of care to patient, family and other interdisciplinary personnel; provides emotional support as needed by patient and family.
- Treats patients utilizing a variety of treatment techniques including (but not limited to): therapeutic exercise, functional activities, Activities of Daily Living, manual techniques, adaptive equipment training, and energy conservation.
- Utilizes age-appropriate teaching methods based upon learning needs of patient; utilizes a variety of teaching tools (models, charts, handouts, etc).
- Evaluates the home, work, and/or community settings and makes recommendations and appropriate referrals to assure safe and efficient function within all settings.
- Evaluates results/effectiveness of Occupational Therapy by observing, noting, and evaluating patients progress; recommending and implementing adjustments and modification of goals and plan as needed.
- Plans and communicates to patient, family and medical personnel readiness for discontinuation of Occupational Therapy in a timely fashion.
- Reviews/finalizes exercise and home programs; initiates and follows up on recommendations for self-management/continuation of care.
- Maintains accurate, thorough and timely documentation in accordance with department policy; chart audit standards are consistently met.
- Demonstrates awareness of regulatory specifications/restrictions/policies; incorporates regulatory information into daily work and operations.
- Responsible for maintaining licensure requirements; practices ethically and is knowledgeable of the Illinois State Practice Act.
- Adapts schedule based on clinic needs; maintains a positive attitude; provides assistance to co-workers and patients; utilizes non-patient care time effectively.
- Contacts referral sources to promote positive relationships with Northwestern Lake Forest Hospital Rehab Services department.
- Actively participates in quality and PI effort.
This position is part-time, 40 hours per pay period, day shift, 2–10-hour shifts (one weekday and every Friday, 7:00 a.m. to 5:30 p.m.)

QualificationsRequired:
- Bachelor's degree.
- Current Illinois OT License
- BLS certification through the American Heart Association or in the process of obtaining certification.
- 0 to 2 years of relevant experience.
