Responsibilities:
Under the direction of the home care manager, the main function of the Occupational Therapist (OT) is to plan and provide occupational therapy program for treatment of patients in a home care setting, including but not limited to, practicing functional prevocational and vocational skills, activities of daily living and preparing progress reports.
- Gathers and reviews patient information; completes initial evaluation of patient, adhering to dept. standards for documentation.
- Continually assesses the effectiveness of treatment program and documents same in visit notes. Makes changes in response to patient’s progress, selecting appropriate techniques and activities to guide patient toward goal achievement. Completes 30-day re-evaluations in a timely manner.
- Collaborates with other health professionals, patient, and/or family in developing discharge plans. Serves as an advocate for patient needs.
- Instructs patient, family and/or caregivers in techniques to be continued after discharge.
- Manages work and patient schedule to make optimum use of time. Adjusts schedule to accommodate unexpected occurrences and volunteers to assist others as needed.
- Routinely documents services provided, to include billing sheets, progress notes, discharge summaries and instructions, home site assessments, and communication with physicians.
- Objectively and thoroughly reports pertinent information at patient related meetings, conferences, and rounds.
- Presents in-services to Home Care Department as assigned.
Qualifications:
- Bachelors Degree required
- Occupational Therapist license in Maryland
- Current CPR Certification
- Prior experience in home health preferred
- Valid driver’s license and reliable transportation
