We are seeking a dedicated Occupational Therapist (OT) to the growing team! The ideal candidate will provide comprehensive occupational therapy services in a Home Health setting. This role involves conducting evaluations, administering treatments, overseeing discharges, and supervising Occupational Therapy Assistants. The OT will create and implement individualized care plans to help patients regain optimal function following injury, illness, or physical impairment. Coordination with assistants is essential to ensure patients achieve their highest level of functional independence.

Key Responsibilities:

• Conduct thorough evaluations to assess patients' functional abilities and needs.
• Develop and implement customized treatment plans aimed at restoring maximum function.
• Perform regular treatment sessions and adjust plans as necessary based on patient progress.
• Oversee the discharge process, ensuring patients and their families are well-prepared for the next steps in care.
• Supervise and collaborate with Occupational Therapy Assistants, providing guidance and support to achieve therapeutic goals.
• Maintain detailed documentation of patient progress and treatment outcomes.
• Communicate effectively with patients, families, and other healthcare team members to ensure coordinated care.

Qualifications:

• Graduate from an accredited Occupational Therapy program.
• Licensed or eligible for licensure by the Ohio OTPTAT Board.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.
