Bonus: Based on weekly production and paid weekly
Occupational Therapy services are delivered in the patient’s natural environment including:
- their home,
- school (preschool, ABA provider, elementary school, etc…)
- community setting (playground, store, public areas)
Skills, Duties, Responsibilities:
- Occupational Therapy Skills
- Detailed knowledge of the scope of occupational therapy
- Current knowledge of treatment practices per discipline and drive to continually learn and grow in the profession
- Provides therapy screenings as needed to support inter-referrals and referrals out
- Case management skills
- High quality case management
- Coordinating case load schedule with the member and their family to support access to care per the treatment plan/plan of care.
- Appropriate utilization and communication with the scheduling administrators to assist in customer care and schedule/case management
- Evaluation planning, conducting, and report writing
- Address client’s aspects of performance to support engagement in occupations that affect health, well-being and life quality
- Assess client’s home and other environments and advice on alterations consistent with their needs and capabilities

- Affectively use clinical observations to complete evaluation details
- Appropriately design long and short-term goals that reflect the unique needs of the patient
- Treatment planning and implementation:
- Select and adapt the appropriate intervention for each member per the occupational therapy treatment model, method and approach to direct the therapeutic process
- Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)
- Teach clients new ways of approaching tasks and educate them on how to break down activities into achievable components
- Recommend adaptive equipment and train in its use
- Documentation
- Ability to produce quality documentation that provides medical necessity for services and continued services
- Timeliness of documentation -within Stellar Home Health policy deadline
- Observe, note, and report on progress of long- and short-term treatment goals
- Customer Satisfaction
- Ability to provide comprehensive and effective parent/caregiver education
- Provide consultations with families and other providers as needed
- Write letters of medical necessity and Home Exercise Programs (HEP) as needed
- Guide and educate family members and caregivers
- Supervision Duties
- Supervise Certified Occupational Therapy Assistant
- Demonstrates proficiency in complying with the Stellar Home Health Therapy Supervisor Standard of Practice
- Scheduling monthly face to face or zoom meetings to discuss their case load
- Giving quality coaching/instruction to the COTA to assure that patients on the COTA’s case load are progressing appropriately and that plans of care are updated and adjusted accordingly
- Timeliness of reading and cosigning all of the COTA’s documentation
- Assuring quality of COTA’s documentation by providing feedback and coaching on documentation
- Completing any needed one on one therapy visits to support the COTA with the therapeutic intervention
- Supervise an Occupational Therapy Student (OTS) or Occupational Therapy Assistant Student (OTAS)
- Proficient utilization and implementation of the Stellar Student program
- Completing all supervision meetings on time
- Providing hands on learning environment for the student.
- Communication with the University/School site placement coordinator regarding the student as needed

- General knowledge of speech therapy scope of practice as it applies to Stellar Home Health
- Responsible for own transportation/vehicle
- Responsible for meeting and treating the patient in their natural environment to achieve the purpose of the home health setting
