An amazing new opportunity has come up on our Patient Care Management! We are looking for a Per Diem Occupational Therapist to join us!

Job Summary: As a member of the Day Habilitation interdisciplinary team, assesses participant’s therapy and developmental skill need areas and recommends goals to meet those needs.

Major Responsibilities:

• Completes an initial evaluation and Service Needs Assessment as it relates to occupational therapy needs for new admissions within 90 days of acceptance.
• Provides OT recommendations for appropriate goals and objectives to be incorporated into the Day Habilitation Service Plan (DHSP).
• Participate in quarterly reviews of participant’s progress, making recommendations as needed.
• Completes a new OT assessment on existing participants every two (2) years, unless there has been a change in status or by the request of the interdisciplinary team.
• Communicates schedule and availability to Health Care Supervisor to ensure proper coverage.
• Completes timely and comprehensive documentation in the form and format provided.
• Other duties and special projects as assigned.

Qualifications:

• Graduate of an accredited school of occupational therapy approved by the American Occupational Therapy Association, Minimum of a bachelor’s degree.
• Current Massachusetts License
• Proficient computer literacy skills, including Microsoft Outlook, Excel, Word, and PowerPoint
• Valid driver’s license and reliable transportation
