Job Details
- Providing and coordinating primary care services for assigned Advantage members
- Developing care plans and ensuring access to preventive care services
- Acting as a clinical resource for providers and facilities
- Collaborating within a team to support individualized care plans
- Engaging with members and designated representatives on care plans and disease processes
Skills You Need:
- Master's Degree in Nursing from an accredited program
- One to two years of clinical experience as a Nurse Practitioner
- Minimum one year in geriatric specialty
- Previous experience in managed care preferred, with NYS NP License, Registration, and DEA certificates required
