About the Job
Healthcare Consulting and Placement is currently hiring bilingual Registered Nurses proficient in Cantonese/Mandarin or Korean. This hybrid/remote position starts with 2 days in our Manhattan office and transitions to 3 days remote after 4-6 months. Qualified candidates must hold an active NYS-RN License and possess bilingual English/Mandarin, English/Cantonese, or English/Korean language skills.

Healthcare Consulting and Placement specializes in providing clinical support and oversight to ensure community members receive safe and effective care.
Key Responsibilities:
Manage service authorizations, create care plans, and coordinate services across the care continuum for high-risk populations.
Assess and coordinate hospitalization/skilled services CHHA/nursing home communication and planning.
Coordinate benefits for Medicare, Medicaid, and other insurances.
Facilitate transitional care management from acute to home, home to skilled nursing facility (SNF)/custodial, and other transitions as needed.
Conduct monthly call assessments and follow up on triggered calls related to medication changes, diagnoses, or critical events.
Coordinate post-discharge services as required.
Qualifications:
RN with BSN preferred; must hold current, valid, and unencumbered NYS-RN License.
Experience in home care, acute care, sub-acute care, long-term care (LTC), or managed long-term care (MLTCP) preferred.
Bilingual proficiency in English/Chinese (Mandarin or Cantonese) or Korean required.
UAS certification and care management knowledge preferred.
