Nursing Home Social Worker - Bridge View Nursing Home

Bridge View Nursing Home, part of The Grand Healthcare System is seeking a Full-time Staff Social Worker to join our team!

Experience in long-term care is preferred!

Duties Include:

• Interviewing participants and their families
• Provide psychosocial assessment and counseling to participant and families
• Developing specific care plans based on the needs of individual patients
• Helping participant apply for appropriate social services
• Providing crisis intervention as needed
• Providing grief and loss counseling and supportive counseling
• Strengthening communications among participant, families, and the program or facility staff

Required:

• Bachelor's Degree

Preferred:

• Long-Term Care Social Work experience 
• NYS Screen Certification 
• Strong Admissions and Discharge Planning experience
