Responsibilities:

As a Nurse Team Leader in Home Care, you will:

• Assume ongoing responsibility for coordinating, implementing, and evaluating the home care needs of patients.
• Review patient insurance information and ensure effective communication of care plans with family members and caregivers.
• Utilize strong communication, documentation, and time-management skills to provide exceptional patient care.
• Precept new nurses and supervise peers during field visits.
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes.

Qualifications:

• Graduate from an accredited School of Nursing; BSN preferred.
• Minimum of 3 years of home care field experience (required); supervisory or leadership experience (preferred).
• Current and valid New York State RN license.
• Valid Driver's License.
• OASIS Certification preferred.
• Knowledge of home health regulations.
• Strong ability to supervise, mentor, and coach home care nurses.
• Proficiency in EMR systems and Microsoft Word.
