Qualifications:

• Graduate of an accredited nursing program
• Two years of experience as an APN, preferably in hospice or home health
• Comprehensive knowledge of pharmacology and palliative care
• Reliable transportation and valid auto insurance

Essential Functions:

• Provide care to Hospice & Palliative patients
• Collaborate with the interdisciplinary team (IDT) for patient evaluations
• Educate patients and families on care processes
• Perform clinical assessments and document visits
• Participate actively in IDT meetings
