Nurse Practitioner or Physican Assistant - Tandym

A health system in Akron, OH, is seeking a dedicated Nurse Practitioner to lead team-based care for individuals with serious mental illness. You will conduct whole-person assessments while collaborating with primary care and behavioral health providers.

Responsibilities:

• Train and advise Medical Assistants and Peer Recovery Specialists on behavioral, clinical, and social health priorities.
• Support care coordination efforts.
• Lead care and disease management initiatives, identifying actionable care gaps.
• Communicate and coordinate with care providers at CMHCs, FQHCs, and hospitals.
• Provide direct patient care when unscheduled needs arise.
• Perform other duties as needed.

Qualifications:

• WA Nurse Practitioner (NP) and/or Physician Assistant (PA) license.

Desired Skills:

• 2+ years of NP/PA experience.
• Experience with behavioral health patients.
