Additional responsibilities include:

• rounding on the high risk patient to prevent readmissions to acute care hospitals
• promoting favorable public relations and representing the Center in a positive light throughout the community, as well as establishing and maintaining positive relationships with residents, relatives, and employees.
• helping to ensure residents maintain their highest level of functioning

NP JOB REQUIREMENTS

• Advanced Nurse Practitioner License
• Excellent interpersonal and verbal and written communication skills
