About the Job
Nurse Practitioners are encouraged to consider an available full-time position with Liberty Healthcare at DHCI - a skilled nursing facility located in Smyrna, Delaware.
In this role, you will focus on providing primary care to a case load of approximately forty older adults and people who are living with various physical and cognitive disabilities.
Your day-to-day responsibilities will include:
- Daily patient evaluation through rounding on units
- Conducting examinations, performing diagnostic tests, providing preventive care, developing and managing treatment plans for acute and chronic diseases, and prescribing medications
- Providing consultation and health education to nursing and ancillary staff
- Collaborating with the facility's Medical Director, nursing team, and ancillary staff
- Actively participating in multidisciplinary treatment team meetings
As a Nurse Practitioner and full-time employee of Liberty Healthcare, you can expect:
- Annual salary between $110,000 and $120,000 (depending on experience)

You are a good fit for this position if you are licensed professionally in Delaware as a Nurse Practitioner and have clinical competence in the care of adult and geriatric patients.
You should be open to serving older adults; people who have loss of cognitive functioning: individuals who have experienced traumatic brain injury; those who are experiencing mental illness; people who have substance use disorders; and those experiencing end of life issues.
