Nurse Practitioner - Bonneville County, Idaho

We are seeking a dedicated and compassionate professional to conduct in-home wellness risk adjustment assessments for Medicare members and other at-risk populations in Bonneville County, Idaho. Job Details

• Employment Type: Permanent, Full-Time
• Salary: $120,000 - $150,000 annually
• Hours Per Week: 40
• Shift Duration: 8 Hour Days

Job Requirements

• Valid and unencumbered Nurse Practitioner License for the state of Idaho
• Minimum of 1 year of experience in patient care (primary care/adult/geriatric, EMR preferred)
• Previous in-home risk assessment experience preferred
• Access to reliable transportation
• Strong ability to work within an EMR system
• Ability to work independently
• Bilingual skills are a plus

Responsibilities

• Perform annual in-home wellness visits and risk adjustment health assessments
• Provide individualized quality care to the elderly population in their homes
• Deliver patient health education on topics such as pain management and medication
• Build relationships of trust with members through exceptional communication and empathy
• Assist in closing quality care gaps (i.e., screenings and labs)
• Collaborate enthusiastically to enhance care delivery
• Travel within the assigned state as required
• Other duties as assigned
