Primary Duties and Responsibilities
The Home Health- Nurse Practitioner is responsible for assisting in the expedient multidisciplinary care of patients for Hoag Hospital and its affiliates, coordinating seamless patient care from admission through discharge in the Home Health setting. Providing supplemental medical care to patients with a focus on quality and patient experience, the Nurse Practitioner can perform history and physical exams, diagnoses and treats medical problems, orders and interprets diagnostic and laboratory tests, performs minor procedures and facilities the discharge process.

Education and Experience
Required:
Master’s degree in Nursing
At least 1-2 years of clinical NP experience
Advanced clinical knowledge and skills in the care and management of patients
Solid computer technical skills required, including proficiency with Microsoft Office products (Outlook, Word, Excel, PowerPoint, and Visio)
Preferred:
Two years recent clinical experience as a Nurse Practitioner, preferably within an ambulatory and acute healthcare environment, related to the care of Orthopedic patients.
Qualified bilingual or multilingual candidates
Licenses Required
Current license in good standing to practice as a Registered Nurse/Nurse Practitioner in California
Furnishing license issued by the State of California and personal DEA number.
Certifications Required
BLS and ACLS certification
National board certification as Nurse Practitioner in area of specialty
