Nurse Practitioner - Family, Adult, or Geriatric
Schedule: Monday - Friday, 8:00am - 5:00pm EST
Location: Philadelphia Metro Area - 45-mile radius from Philadelphia city center
Type: 100% In-Home Visits/In the Field
Job Responsibilities
Perform home visits with patients enrolled in In-Home Primary Care within a 45-mile radius of Philadelphia city center
Conduct comprehensive evaluations during initial visits, targeting risk factors for disease exacerbations and hospitalizations in subsequent visits
Act as, or collaborate with, Primary Care Providers (PCP)
Coordinate patient care with other providers, including specialists and health plan case managers, to ensure cohesive care
Work closely with care team members to deliver quality care in a timely manner
The care center will assist with scheduling and triaging urgent issues
Share after-hour telephone on-call coverage with other licensed providers
Collaborate with field and office RN case managers and medical assistants
Document acute and chronic healthcare issues, symptoms, and goals of care in the EMR system
Ensure documentation is thorough and completed in a timely manner
Obtain appropriate consent forms, signed and dated
Engage frequently with patients, coders, schedulers, and supervising physicians
Educate patients and/or families on chronic medical conditions, preventative care, and medication adherence
Maintain professionalism at all times
Communicate with colleagues in person, virtually, or through other professional methods
Attend mandatory meetings
This role requires driving within a 45-mile radius of Philadelphia, PA
Qualifications
Prior clinical experience in primary care, palliative care, oncology, or geriatrics preferred
Experience in home-based care preferred
Ability to work in diverse home environments
Proficiency with electronic medical records and various computer-based applications such as Microsoft Office
Strong background and interest in primary care and preventative medicine
Valid state driver’s license required
Self-directed, highly motivated professional with strong organizational skills and comfort working independently daily
Excellent clinical assessment, analytical, leadership, and interpersonal skills
Effective problem resolution and professional demeanor in difficult situations
Excellent verbal and written communication skills
Demonstrated ability to identify, diagnose, treat, and document acute and chronic disease in adults
Proven ability to recognize conditions, characteristics, and behaviors that place patients at risk
Commitment to pursuing feedback, continuing education, and professional development
