This is a PRN position.
Position Summary:
The Nurse Practitioner assists in performing Face-to-Face Encounters as needed in patients' homes, nursing homes, and inpatient facilities to determine continued eligibility for Medicare hospice benefits. The Nurse Practitioner will collaborate and cooperate with the Peoples Hospice Medical Director and will not certify the terminal illness.
Requirements:Essential Duties & Responsibilities
- Responsible for performing Face-to-Face Encounter for the Medicare Hospice Beneficiaries starting their third benefit period and beyond.
- Collaborates with the RN Case manager and/or RN Patient Care Coordinator to determine which patients' the Nurse Practitioner will perform the Face-to-Face Encounter on and the time frame in which the encounter will be completed.
- Consults with the team members on a PRN basis for clinical information relating to the patient/family.
- Reviews the patient records and pertinent medical information prior to the FaceĀ to-Face Encounter.
- Documents the clinical findings from the Face-to-Face Encounter on the appropriate forms as provided by the hospice.
- Ensures that the clinical findings of the Face-to-Face Encounter have been provided to the certifying physician for use in determining continued eligibility for hospice care.
- Adheres to company policy regarding protected health information.
- Assures the accuracy and timeliness of documentation, including bills and clinical notes.
- Represents values and mission of Peoples Hospice within the professional and lay community.
- Other duties as assigned by the VP of Operations and Medical Director.
Required Qualifications:
- Registered Nurse duly licensed to practice as an Advanced Practice Nurse in the State of Florida under the provisions of the Florida State Board of Nurse Examiners and accreditation from an approved certifying body for advanced practice nursing (American Academy of Nurse Practitioners or American Nurse Credentialing Center).
- Comprehensive knowledge and well-developed skills in:
- Medicine and Pharmacology
- Pain and Symptom Control
- Psychology of loss
- Valid Florida Divers License and automobile insurance.
- Understanding and acceptance of Peoples Hospice and Palliative Care principles of care.
Work Environment
While performing the duties of this position, the employee travels by automobile and is exposed to changing weather conditions. The employee works in varied residential and facility environments based on assigned caseload.
Physical Demands
While performing the duties of this job, the employee is required to see, talk, or hear. The employee is regularly required to stand, walk, sit, reposition patients; use hands and fingers to feel objects, tools, or controls; and reach with hands and arms. The employee frequently lifts and moves up to 50 lbs.
