responsible for being a capable clinician who hasacquiredbroad experience in caring for patients and has developed a sound understanding about the care of a particular patient population and routinely usesacquiredknowledge, theory,researchand experience to respond to changes in clinical situations.This RN professional can apply the Synergy Model for Care in practice and use this knowledge to personalize care for each patient/family while serving as a resource to colleagues and strong advocate for them.This individual will: JOB DESCRIPTION: The Nurse Practice Specialist is the clinical and education lead for a program or an area of defined nursing practice. Provides leadership in order to integrate new knowledge and innovation within a specialty area and appreciates system levels to help close the gap ensuring healthcare delivery is safe, timely, effective, evidence-based, equitable, efficient, and patient/family focused. Inter professionally collaborates with and provides expert consultation to administrative leaders, educators, nurses, physicians to lead clinical practice to promote and maintain quality improvement to improve patient and population health outcomes on the unit, within the service line, and across the organization. Contributes to the advancement of nursing practice at the unit/system, local, state, national, and/or international level and is instrumental in the application and lead of the Professional Practice/Care Transformation Model across the health system. Participates in and leads organizational and hospital-based professional activities to maintain and/or contribute knowledge of current trends, practices, and developments. Directs and manages process improvement projects and initiatives that are congruent with EHC and nursing's strategic plan for innovation in healthcare, interprets results, disseminates findings, and utilizes technology and clinical and operational data for the improvement and transformation of patient-centered care modeling transformational leadership behaviors. Influence and guides direct care for complex patients with multi system issues including the patients/families biophysical, psycho-emotional, and spiritual issues at the individual and community level and evaluates all processes to determine effective implementation and outcomes needed to meet patient goals and attain optimal care delivery outcomes. Research & Publications The Nurse Practice Specialist provides guidance in: Improving the health of individuals, families, and communities by preparing nurses and other healthcare team members to identify social determinants of health, as well as identifying the system facilitators and barriers to the provision of effective, efficient, equitable, and accessible care for all across the care continuum. Considers cultural influences and is cognizant of the diverse needs of patients, families, staff, and other professionals. Demonstrates leadership in ethical decision- making and patient advocacy. Engages in ongoing self-appraisal, seeking feedback from colleagues, other professionals, patients, and families. Leads implementation in the principles of fair and just culture. Utilizes coaching to facilitate optimally functioning staff and multidisciplinary teams applying advanced communication skills. Mentors and coaches professional development; including collaboration with resident nurses on their evidence based projects and staff seeking DNP degrees. MINIMUM QUALIFICATIONS: Professional Nursing Certification required (CCRN, CEN, OCN, etc.), Doctor of Nursing Practice (DNP) and three years of clinical nursing experience as a Registered Nurse with at least two years of recent experience in the area of concentration. BLS and ACLS certifications required.
