East Bay Community Action Program (EBCAP) is seeking a full time Nurse Manager for our 6 John H. Chafee Boulevard in Newport, RI. The Nurse Manager position will report to the Director of Nursing and collaborate with other members of the Health Center leadership team to promote integrated care and provide seamless operations of the health center. We are seeking a highly professional and motivated individual who enjoys working with clients in a community health setting. Join our expanding dynamic multidisciplinary team and provide comprehensive patient care using a care team model.
The Nurse Manager will provide approximately 50% of his/her time in direct, clinical support to the medical providers and/or behavioral health prescribers working within a Patient Centered Medical Home (PCMH), and/or Integrated Home Health (IHH) Team, while the additional 50% will be administrative to support the clinical care team staffs (i.e., Nurses, Lead Medical Assistant, etc.). The Nurse Manager will work collaboratively with clinical and administrative staff to ensure enhanced clinical care, superior customer service, and adherence to various licensing and accreditation standards and quality improvement initiatives. The Manager will lead training initiatives, embrace patient advocacy, and be the “go to” for clinical staff in the Health Center.
The successful candidate will have a minimum of an Associates’ Degree in Nursing or equivalent and be licensed as a Registered Nurse by the Rhode Island Department of Health, as well as certified in Basic Life Support (BLS). Two or more years’ experience in a community health setting, acute care or emergency/urgent care working with primary care providers to coordinate care and disease management required. Previous experience in psychiatric nursing a plus. Previous experience in an electronic medical record (EMR) preferred; however, would receive training in EBCAP’s EMR which is NextGen.
