Full job description
The LPN/RN is responsible for providing direct patient care under the supervision of the Director of Nursing or physician. The LPN/RN will be responsible for administering medications, taking vital signs, performing wound care, and assisting patients with daily living activities. The LPN/RN will also be responsible for documenting patient care in the electronic health record system and liaising with other healthcare professionals as needed.
Responsibilities
- Administer medications as prescribed by a physician or Director of Nursing
- Monitor and record patient vital signs, such as blood pressure and heart rate.
- Communicate with patients and their families about their care and treatment plan.
- Document patient visits and treatment plans in the electronic medical records system
- Collaborate with other healthcare professionals, such as registered nurses, physicians, and therapists.
- Complete patient assessments, coordination of care
- Identify and report changes in the resident’s condition.
- Follow infection control and safety protocols to ensure patient and staff safety.
- Maintain confidentiality and ethical standards in all patient interactions.
- Other duties as assigned by management.
Requirements
- Valid Illinois Nursing License LPN or RN.
- Experience in long-term care or healthcare setting preferred.
- Excellent communication and interpersonal skills.
- Strong attention to detail and ability to follow instructions accurately.
- Ability to arrive on time for all scheduled shifts.
- Must have the ability to read, write, and follow oral and written directions at a level necessary to accomplish the job.
- Detail-oriented with strong organization and prioritization skills.
