Nurse Liaison

Key Responsibilities:
- Act as the primary point of contact for patients and families, offering guidance, support, and information throughout their healthcare journey.

- Coordinate with hospice or home health agencies to arrange services and support for patients transitioning to home or hospice care.

- Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with regulatory standards.

- Stay informed about hospice or home health policies, regulations, and best practices, continuously enhancing your knowledge and skills.
