NP and PA Needed on the Big Island of Hawaii - Hilo Medical Center

This position is for a Nurse Practitioner (NP) or Physicians Assistant (PA) at the East Hawaii Health Clinics, a department of Kau Hospital of the East Hawaii Region on the island of Hawaii. The function of the East Hawaii Health Clinics is to provide outpatient primary, specialty, and urgent care services. This provider will function independently, performing primary care and urgent care services to all patients of the East Hawaii Health Clinics within their scope of practice.

Most importantly, this provider will help develop and implement a Chronic Care Management (CCM) program, working with chronically ill or high-risk patients with the goal of enhancing patient health and the ability for self-management and shared decision-making. Other program goals will include reduction in emergency room utilization and hospital readmissions. This provider will be part of a CCM team that will assess and address patients unmet health and social needs, assist patient with development of treatment goals, and help patient negotiate cognitive, physical, social, and economic barriers to care.

• Patient education
• Outreach
• Referral across a network of services.

Care will be delivered in a variety of settings, including home visits, and there may be evening and weekend hours
