PRACTICE SPECIFICS:

The Nurse Practitioner utilizes advanced health assessment, knowledge, and decision-making skills, while working both independently and in collaboration with physicians and other health care professionals. The NP will provide health care to individuals and families, with a focus on providing high-quality, person-centered end-of-life care. The position requires a high level of customer service skills to establish and enhance positive and trusting relationships with patients, caregivers, and co-workers.

• This position will be 40 hours per week with rotating evening and weekend call.  Pay stipend for night and weekend call above the base pay rate.
• The Hospice position includes visits to patients in their place of residence (private home, skilled nursing facility, assisted living etc.), rounding at the Solvay Hospice House, and providing support and education to hospice interdisciplinary team members virtually, in-person, and over the telephone.
• Travel is required for home visits. Mileage is compensated at the federal rate.
• The ideal candidate is passionate for end-of-life care, thrives in a team environment but also possess the ability to work independently, brings a positive attitude, has excellent interpersonal communication skills, is willing to learn, be flexible, and support their colleagues.  
• Outreach to Virginia and Deer River, MN.
• Provides care to patients in MN and WI

EDUCATION:

Master’s or Doctorate degree in Nursing from an accredited program.

LICENSURE/CERTIFICATIONS REQUIRED:

• Current licensure as a NP in MN and WI (or must apply for WI licensure within 3 months of hire)
• Current certification in CPR
• National certification, in applicable area
