New Journeys Case Manager

Our Case managers work with clients ages 15-40 who are experiencing their first episode of psychosis.  This position is a vital role providing stability and support during a difficult time in their lives.  You will work alongside clients to engage them in taking charge of their own health care, provide clinical and resource screening and assessments, and help coordinate services among their healthcare providers and support groups.  As a support and advocate for your client, you will visit with them regularly and attend health provider appointments as needed.

What we’re looking for:

• One of the following:
  • Bachelor’s Degree in Social Work, Psychology, Human Services, Behavioral Sciences, or a related field
  • Certified Medical Assistant with an Associate Degree
  • Certified Community Health Representatives (CHR) or DOH Certified Chemical Dependency Professional
• 2 years or more of experience working in a nursing, health care, behavioral health, or human services capacity (paid or volunteer)
• Ability to pass background checks
• Valid Driver’s license in good standing, and current auto insurance
