About the Job
As our Navigation Specialist, you will provide information, assistance regarding Mental Health Partners (MHP) services, access to services, program information, referrals, and financial resources for economic barriers to care. You will collaborate within MHP and the community to help coordinate basic client needs related to food, transportation, healthcare, medical resources, and other community benefits
You will rely on your knowledge of services and resources within our organization and throughout the Boulder and Broomfield communities we serve. You will serve individuals to connect them with the services and resources to best support quality care This is hybrid position that will be based in our Boulder, CO office. You will report to the Program Manager.
You will primarily be remote in the Denver Metro area and you would need to be able to come into our locations on occasion for on-site trainings or for potential coverage.
Job Profile:
You will welcome the clients over the phone, refer to agencies and get clients started with services
You will determine when to refer clients elsewhere, based on your assessment of the right for the client based on their needs
You will provide a proper, warm handoff to other organizations to help the client seek services
You will provide an intake to learn about the client, assess their needs and provide client care and clinical support

What we need for this job:
Two (2+) years of experience, mental health environment
Bilingual Spanish
Experience with healthcare environments and their complexities
