About the Job
Our Client, a Medical Centre company, is looking for a MSW Social Worker - Hematology (Sickle Cell- Pedi and Adult) for their Boston, MA location.
Responsibilities:
- Conduct comprehensive psychosocial assessments to identify patient and family needs, strengths, and resources in both inpatient and outpatient settings.
- Develop and implement individualized care plans, including psychosocial diagnoses and tailored interventions, to support patients' medical and psychosocial goals.
- Provide counseling, crisis intervention, and emotional support to patients and families dealing with the acute and chronic impacts of SCD, including pain management, treatment adherence, and care transitions.
- Round with interdisciplinary inpatient teams to ensure seamless integration of psychosocial support into patient care plans.
- See patients in outpatient clinics, providing ongoing support, education, and advocacy to address the long-term management of SCD.
- Facilitate education for patients and families to enhance understanding of SCD, promote self-advocacy, and foster resilience in managing their condition.
- Collaborate with physicians, nurses, case managers, and other interdisciplinary team members across inpatient and outpatient settings to deliver holistic care.
- Serve as a liaison with external agencies, such as social service organizations, schools, and community resources, to advocate for patients and secure access to necessary supports.
- Consult with legal and governmental entities, including courts and public assistance programs, to address systemic issues impacting patient care.
- Provide mentorship, education, and coaching to patients, families, and team members to improve care outcomes and patient satisfaction.
- Participate in program development, quality improvement initiatives, and efforts to enhance the continuum of care for patients with SCD.
