For detailed information on Benefits for this position, click here ( https://hr.uw.edu/benefits/wp-content/uploads/sites/3/2018/02/benefits-classified-staff-2022-V1\_a11y.pdf) **HARBORVIEW'S STREET MEDICINE OUTREACH PROGRAM** has an outstanding opportunity for a **MENTAL HEALTH PRACTITIONER** **Work Schedule** + 80% FTE, 32 hours per week + Alternate Shift + Friday - Sunday 9 hour shifts, Monday 5 hours **POSITION HIGHLIGHTS** + Opportunity to work with an underserved, disenfranchised homeless population, reduce health disparities, and improve health outcomes + Join a multi-disciplinary team (MD, ARNP, RN, Community Health Worker, Program Coordinator) to deliver integrated, patient-centered care to our mostvulnerable and underserved neighbors **DEPARTMENT DESCRIPTION** The Pioneer Square Downtown Programs focus on providing care for Harborview's mission population to address health equity issues by increasing access to care in our unique primary care settings, homeless shelters, housing programs or community setting for people experiencing homelessness or are vulnerably housed. This is a unique opportunity to be a part of a collaborative approach in client-centered care provision. This multi-disciplinary team will provide outreach, engagement, and direct care aimed towards our unhoused population within the Seattle area including but not limited to encampments, tent cities, streets, under bridges, shelters, etc. We will also receive and respond to referrals from Harborview Medical Center's Emergency Department, inpatient units, and Medical Respite program to support transitions of care for our unhoused population. **JOB RESPONSIBILITIES** + Conduct outreach, engagement, and direct care for unhoused individuals in various environments, such as those living in their cars, encampments, tentcities, streets, under bridges, and in the woods + Participate as a member of a multi-disciplinary team to serve people experiencing homelessness, including those with substance use disorders, mentalillnesses, and complex physical health conditions + Perform psychosocial and social service needs assessments for patients, connecting them to relevant community resources and supporting care coordinationand linkages + Deliver behavioral health and substance use support, including screenings, treatment, harm reduction resources, and connections to ongoing care asappropriate + Develop cooperative relationships with other agencies in the medical, mental health, and social service systems in order to provide effective, wraparoundservices + Act promptly and effectively in managing self, patients, and other employees during emergency situations + Document patient care in a timely manner that reflects recognition of the legal significance of an accurate and complete mental health record **REQUIREMENTS** + A master's degree in behavioral sciences, such as psychology, psychiatric nursing, occupational therapy, vocational or educational counseling, or social work + OR Bachelor's degree and at least three years experience in the delivery of case management services, crisis intervention, suicide assessment, resource + Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration **ABOUT HARBORVIEW MEDICAL CENTER** As the region's only Level I Trauma and verified burn center, Harborview Medical Center is a comprehensive healthcare facility owned by King County and operated by UW Medicine.
