Home Health & Hospice Social Worker
Job duties, but not limited to, utilizing professional standards to ensure safe, quality coordination and provision of social services to patients in pediatric, adolescent, adult, and geriatric age groups in the Home Health and Hospice setting while maintaining compliance with regulatory requirements, HIPAA, and KDH policies and procedures. Providing screening assessments of identified individuals in order to assist with resource management, care transitions, crisis management and interventions connecting patients and families with appropriate resources. Bereavement support; Provides counseling services when necessary and within scope of training; Participates in rotational call; Collaborates to ensure optimal quality of care through timely coordination of health care services with members of the health care team within the organization and community agencies; Expedites access to the most appropriate level of behavioral health; Assist persons who are experiencing a variety of acute financial/support needs in the attainment of needed resources while taking into consideration financial, social and clinical factors; Initiates Adult and child Protective Services reports;
Requirements: Applicants must have a minimum of a Master's of Social Work (MSW) degree or a Baccalaureate degree in Social Work (BSW) degree from a school of social work accredited by the Council on Social Work Education (CSWE) and an active Indiana license as a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) and one year health care experience. Must have current driver's license and own transportation.
