Vivo HealthStaff is recruiting for a Medical Social Worker in Fremont for a wonderful established healthcare organization. This is a full-time position with salary and benefits.
Position Overview:
The Social Worker is responsible for providing direct case management services to patients. The main function of this role is to support patients and their families/caregivers in dealing with social, emotional, and practical aspects necessary for maintaining independence. Under the guidance of the Social Work Lead, the Social Worker acts as a key component of the Interdisciplinary Team (IDT), offering assessments, coordination, treatment planning, information, referral to community resources, and other social work services to address the complex needs of patients and their families.
Key Responsibilities:
Clinical Practices and Case Management
- Exercise independent judgment and discretion in managing a caseload of patients.
- Perform initial and ongoing psychosocial assessments to develop, implement, and update tailored care plans and interventions.
- Collaborate with caregivers and support systems in care planning and service delivery.
- Identify and address risks and other psychosocial issues.
- Determine each patient's service needs; advocate, refer, coordinate, and follow up promptly.
- Provide individual counseling and facilitate family meetings.
- Engage with broader community resources to fulfill specific patient needs.
- Conduct discharge planning upon a patient's exit from the program.
- Lead patient and/or caregiver groups and contribute to events like weekend caregiver workshops.
Interdisciplinary Team Member Roles and Responsibilities
- Serve as the primary link between patients, caregivers, the team, and the community.
- Collaborate with internal departments and community agencies for comprehensive care.
- Uphold all ethical and legal standards of social work practice.
- Follow all clinic policies & procedures, including those for confidentiality and safety.
- Ensure timely and accurate documentation.
- Engage actively in team meetings, advocating for patients and caregivers, resolving team differences, and supporting team plans.
- Promote and implement exceptional service standards.
- Participate in the enhancement of social work services and system efficiency.
Qualifications:
- Master's degree in Social Work from an accredited institution.
- At least one year of experience working with the frail or elderly population.
- Capable of serving a diverse patient base.
- Proficient in interdisciplinary team collaboration.
- Excellent interpersonal, organizational, and follow-up skills.
- Strong written and verbal communication abilities.
- Competence in Microsoft Office and general computer literacy.
- Bilingual skills may be needed for specific roles.
- Preferred: Experience with electronic health records, one year of post-MSW experience, case management, and healthcare experience.
Working Conditions:
- Fieldwork in patient homes and community settings may be required.
- Travel to other locations for meetings or training may be necessary.
